In Canada, not everyone has equal access to palliative and end-of-life care. While the Worldwide Hospice Palliative Care Alliance called on governments to make equity a priority by addressing the structural and social conditions that create unfair differences in care, many populations are still underserved in BC.
(For more information about how BCCPC is working to address these inequities, read about our collaboration on Facilitating Equitable Access to Palliative and End-Of-Life Care for Underserved Populations in British Columbia: Knowledge Exchange Series.)
We are building a comprehensive list of advance planning resources (ACP) for all of the underserved populations identified in the Knowledge Exchange, which will be added to our Advance Care Planning Hub in 2026.
In the meantime, here is a sample of some of the population-specific ACP resources available:

2SLGBTQ+ populations: Planning for My Care
Indigenous populations: Advance Care Planning: Preparing a Basket with What Matters to You

Vulnerably housed populations: Equity-informed advance care planning tool
