Pilot Phase (October – February 2021)
The project partnered with four community-based organizations to pilot the guided group conversation event and ACP information sessions from October 2020 to February 2021. Our community partners received a comprehensive toolkit of resources and the project team provided training for up to four staff/volunteers from each organization. A total of eleven (11) individuals were successfully trained to become ACP facilitators for people living with early stages of dementia and their family/friend caregivers.
Pilot Evaluation and Toolkit Refinement
Evaluation data was collected from organizers, facilitators, and public participants to inform toolkit refinement and improvement. The update curriculum and toolkit will be tested in the implementation phase of the project (March – June 2021).

Newly launched in Spring 2020, the Adult Cognitive Wellness Centre uses Montessori principles to deliver cognitive, physical, social, spiritual and emotional programs to their clients living with dementia, while also supporting and engaging their caregivers. Learn more here.

While their primary mission is to provide quality end-of-life care to the North Okanagan community, the Society’s REACH program (Resources, Education and Advocacy to Champion Hospice-palliative care) demonstrates their belief that palliative care starts long before people enter hospice. Rather, it starts at home, around a kitchen table, talking to loved ones about end-of-life wishes. Learn more here.
More than providing pharmacy services to residents in long term care, retirement homes, hospice homes and group homes in Greater Vancouver, CareRx also focuses on education – their own and their clients’.
