DESCRIPTION:Event Hosted by: Canadian Palliative Care Nursing Association (CPCNA) \nEvent cost: Free to members\, $40 to non-members \n\n\n\n\nHow can a patient with advancing heart failure (HF) experience care that alleviates suffering and helps them to live their best life? While some progress has been made to integrate a palliative approach to the care of these patients and their families\, challenges exist in bridging the two distinct care cultures of HF and Palliative Care. Essential to this important practice shift is the effective communication between health care providers\, patients and their family caregivers. This presentation will explore how the Heart Failure Palliative Approach to Care (HeFPAC) was designed to build capacity for care through a shared language that fosters communication about priority issues that commonly arise in advanced HF. \n\nPresenter Bio: \n\n\n\n\nPatricia Strachan RN MSc PhD is an Associate Professor in the School of Nursing at McMaster University. Dr. Fromme will give a short overview of the Serious Illness Care Program’s COVID-19 Toolkit and Dr. Mitchell will describe how Boston Medical Center has implemented elements of the COVID-19 Toolkit as part of their Pandemic response. Next\, Dr. Fromme\, Ms. Joyner\, and Ms. Miltz will discuss best practices and cases for serious illness conversations during telehealth encounters using both phone and video platforms. \nRegistration (link below) is required.\nPasscode: community \nMeeting Registration – Zoom \n  \nThe Serious Illness Care Program’s COVID-19 Toolkit was developed in Spring 2020\, during the first peak of the pandemic in the United States. It includes four conversation guides adapted to the context of COVID-19 for use in inpatient\, outpatient\, long-term care and crisis standards settings. There are video demonstrations of clinicians using the guides\, as well as tools to support implementation and patient-facing resources. \nWebinars are presented live\, and are recorded.
