DESCRIPTION:Event Hosted by: Canadian Palliative Care Nursing Association (CPCNA) \nEvent cost: Free to members\, $40 to non-members \n\n\n\n\nHow can a patient with advancing heart failure (HF) experience care that alleviates suffering and helps them to live their best life? While some progress has been made to integrate a palliative approach to the care of these patients and their families\, challenges exist in bridging the two distinct care cultures of HF and Palliative Care. Essential to this important practice shift is the effective communication between health care providers\, patients and their family caregivers. This presentation will explore how the Heart Failure Palliative Approach to Care (HeFPAC) was designed to build capacity for care through a shared language that fosters communication about priority issues that commonly arise in advanced HF. \n\nPresenter Bio: \n\n\n\n\nPatricia Strachan RN MSc PhD is an Associate Professor in the School of Nursing at McMaster University. She is Chair of the Canadian Association Schools of Nursing Palliative and End-of-Life Care Interest Group and Co-Chair of the Healthcare Communication in Nursing Special Interest Group for the European Association for Communication in Healthcare.
