DESCRIPTION:Session resources\n\nSession summary notes\nSession recording: YouTube\n\nAdvance care planning can be more complex with adults who have cognitive or communication differences. Understanding concepts of inclusion\, competency and a model of shared decision making can be helpful for the clinician in supporting care planning and medical directives that align with the individuals wishes.
