All Together, a compassionate communities initiative, is open to Health Care Providers, Volunteers, Volunteer lead organizations, Community Groups, and individuals looking to be inspired by and learn from others developing compassionate communities.
Currently, we ask people through a medical lens to tell us about their advanced care plan and what they want as they move towards death. This can cause stress and uncertainty, especially when death is not imminent. Instead, by connecting earlier through curiosity (asking What Matters To You), we can build trust. Through listening, we can use our understanding to guide someone’s health journey to best fit their needs and wants. Come join the conversation!
